The hazard
THE WELDS ATTACHING THE HOIST LIFTING BRACKETS TO THE DUMP BODY COULD FAIL ALLOWING THE DUMP BODY TO DROP TO THE TRUCK FRAME WHILE PUSHING THE HOIST FORWARD INTO THE CAB OF THE TRUCK. THIS COULD RESULT IN INJURY TO THE CAB OCCUPANTS. CORRECTION: BRACKET PLATES WILL BE DRILLED AND BOLTED TO THE BODY TO PREVENT BREAKING OF THE WELD. NOTE: VEHICLES LOCATED IN ONTARIO.
